Demographics

HBN collects a medication history as well as any previous hospitalizations and all prior treatment information. HBN also collects demographic information such as gender, ethnicity, and education.

Note: The Demographics Questionnaire is administered to all adult participants age 18+ and to parents of participants ages 5-17. The questionnaire was developed by CMI specifically for the HBN study.
